How To Access Your Medical Records
To access your records of care at The Counseling Center, you can:
Send an email to recordrequest@thecounselingcenter.org
Call 740-464-4635 on weekdays M-F, 8 am- 4 pm to reach our EHR Admin/Manager.
Send a fax to 740-529-0549 to request records.
Required Documentation
The Counseling Center must have a release signed and dated. All releases must cover 42 CFR Part 2. If you are without the appropriate documentation, the two listed below can be used.
